Access, then a thorough clean
Deep pockets between gum and tooth cannot be reliably cleaned from the top down — instruments simply cannot reach the bottom of them, and disease continues underneath. Flap surgery lifts a section of gum so the root surfaces and the bone around them are directly visible.
The area is cleaned thoroughly, any irregular bone contour is smoothed, and the gum is repositioned and sutured so it fits closely against the tooth. The aim is to leave pockets shallow enough that you can keep them clean yourself — which is what determines whether the result holds.
